Filling out your requisition form
Most Doctor’s Data tests require:
- Sample collection date and time
- Sample type (urine, blood, stool, saliva)
- Additional test-specific context (menstrual status, supplements used, etc.)
Review the table to see which fields you must complete for your particular test:
Test type: Required fields
Blood panels: -Collection date and time-Fasting status
Urine elements: -Collection date and time-Volume-Whether part of a challenge (if applicable)
Hair: -Sample type-Hair treatment history-Shampoo used
Saliva (hormone panels, CAR): -Collection date and time-Wake-up time-Menstrual status-Storage confirmation
Neuro hormone & neurotransmitter: -Collection date and time-Height-Weight -Menstrual status
Cardio/Metabolic: -Collection date and time-Height-Weight-Fasting confirmation
For tests involving pre/post-loading or challenge protocols, your provider should advise you on what to document. If you’re unsure, contact them directly.
